Radiation Dose to Oesophagus in Breast Cancer Patients Receiving Radiation Therapy: A Retrospective Study

Background: Breast cancer is the most common cancer in women worldwide. Management of Breast cancer requires multidisciplinary team approach (viz, Surgery, Chemotherapy, Radiation therapy, Hormonal therapy & Targeted therapy). Radiotherapy reduces the risk of breast cancer mortality for women after breast conserving surgery and for women after mastectomy for node-positive disease. One of the organs at risk in breast cancer therapy is esophagus. As such, there is potential to expose greater volumes of esophagus to radiation. This may result in increased frequency and severity of acute radiation esophagitis during treatment. Aims and Objectives: To evaluate dosimetric parameters such as mean dose (Dmean) maximum dose (Dmax) which may influence the incidence and severity of esophagitis in breast cancer patients receiving radiotherapy. Material and Methods: Treatment plans of post mastectomy patients who had already received adjuvant RT of dose 50 Gy in 25 # over 5 weeks to the chest wall along with Supraclavicular field (SCF) using 3-Dimensional Conformal Radiation Therapy (3D CRT) and Intensity Modulated Radiation Therapy (IMRT) were selected. Mean dose (Dmean) and maximum dose (Dmax) to esophagus was assessed. Results: Dosimetric parameters assessed are Dmean and Dmax. Dmean assessed up to less than or equal to 11Gy and Dmax assessed upto less than or equal to 34Gy. The average of Dmean in this study is 8.34Gy which is within normal limits of prescribed dose. The mean Of Dmax in this study is 24.80Gy which is in limits of prescribed dose. Conclusion: Increased dose to esophagus in observed when IMRT is used especially when supraclavicular or internal mammary nodes are involved. Routine contouring of esophagus and planning accordingly may reduce oesophageal dose and acute oesophageal toxicity.

A Review Study on Gender Determination with the Help of Bite Marks Analysis

Bite marks are commonly observed in cases such as slaughter, sexual assault, child abuse cases, and during sports events. Bitemark is a type of ‘patterned injury’ which played an important role as evidence in judicial system. Bite marks can find on various body parts and various edible leftovers at the Crime Scene which can be used as evidence for identification of the criminals. Teeth impression shows sexual dimorphism and therefore helps in gender estimation. The various dental characteristics are individual-specific. A piece of exhaustive information and examination of indentations can help in the vindication of the guiltless as well as give decisive proof to the conviction of criminals. In certain violations, indentation proof is the main proof on which conviction has been accomplished, especially in rape and kid abuse cases. The present review describes the importance of bite mark analysis, estimation of gender through bite marks, and stability of teeth impression in diverse conditions. This article can give a better comprehension of the methodology used in forensic dentistry when investigating dental impressions.

Suicidal Poisoning by Ingestion of Taxus Baccata Leaves: A Case Report with Review of the Literature

Taxus baccata is one of the most poisonous trees in the world. It contains taxanes and toxic alkaloids: the most dangerous one is the taxine. It is a fairly common plant in our environment and could be extremely dangerous; however, yew poisoning is quite rare. Cases may occur by voluntary or accidental ingestion. Therefore, the rapid orientation and diagnosis of suspected episode are important, in order to treat it quickly.

Isolated In-Situ Axial Hepatic Mal-Rotations: Implication on Laparoscopic; Surgical Anatomy and Practice

Three dimensional anatomy and congenital anomalies are crucial determining the trocar positions and the optimal site view for any laparoscopic procedure. Such critical issue mandates preoperative diagnosis of topographic anatomical variations. Clinical reporting on congenital foregut anomalies does not include yet variation in liver rotation. This research is documenting such unrevealed anomaly and stressing on its implication in the field of contemporary laparoscopic surgery.

Interrelationships between Body Weight and Dimensional Shell Measurements of Giant African Land Snails (Archachatina marginata) in Calabar, Nigeria

Two hundred Archachatina marginata snails, one hundred each of A. marginata var. saturalis and A. marginata var. ovum with weights ranging from 33.10 – 349.00 g and 127.60 – 443.40 g, respectively sorted out of a base population were used for this study. Phenotypic traits measured on each of these snail varieties/strains included shell length, shell width, aperture length, aperture width, spiral length, spiral width, diagonal length, length between aperture and first spiral, number of whorls and body weight. Data collected were used to estimate phenotypic correlations between pairs of traits and to predict the relationship between body weight and other dimensional shell measurements (DSM). Results of phenotypic correlations between body weight and the DSM and between the DSM of the two A. marginata varieties showed that all the pairs of phenotypic traits investigated on A. marginata var. ovum expressed positive correlation values, while the pairs investigated on A. marginata var. saturalis showed both positive and negative correlation values. The regression estimates of parameters and coefficients of determination for the simple linear function of A. marginata var. ovum snails showed slightly high and very strong interrelationship between body weight and one phenotypic trait, while the multiple linear function for predicting body weight using four phenotypic traits showed highly significant and very strong interrelationship. In A. maginata var. saturalis snails, both the simple and multiple linear regression equations showed highly significant and very strong interrelationships between body weight and shell parameters. The range values of coefficient of determination showed that 78 to 100% of the variability in both snail strains body weights can be explained by changes in the considered dimensional shell parameters. Also, the linear functions with four and two parameters/traits best predicted the live weights of A. marginata var. saturalis and A. marginata var. ovum snails, respectively. Prediction results showed that explainable traits best predicted live weight when more than one phenotypic/shell trait were fitted into the regression functions.

On Similarity of Pressure Head and Bubble Pressure Fractal Dimensions for Characterizing Permo-Carboniferous Shajara Formation, Saudi Arabia

Pressure head was gained from the distribution of pores to characterize the sandstones of the Shajara reservoirs of the permo-Carboniferous Shajara Formation. The attained values of pressure head were employed to calculate the pressure head fractal dimension. Based on field observations in addition to the acquired values of the pressure head fractal dimension, the sandstones of Shajara reservoirs were divided here into three units. The obtained units from base to top are: Lower Shajara Pressure Head Fractal Dimension Unit, Middle Shajara Pressure Head Fractal Dimension Unit and Upper Shajara Pressure Head Fractal Dimension Unit.

Xeroderma Pigmentosum Groups C and A in Algerian Patients with Deregulation of both Transcription and DNA Repair

Xeroderma Pigmentosum (XP) is a rare autosomal recessive disorder characterized by an extreme sensitivity to UV rays from sunlight, a high incidence of skin cancer and occasional neurological symptoms. XP, primarily defined as a DNA repair syndrome, has been found associated with defects in the Nucleotide Excision Repair (NER) pathway, and more recently by transcriptional deregulation. XP results from mutations in eight genes (XPA to XPG and XPV) coding for proteins involved in NER.

Tomotherapy Treatment of Chronic Lymphocytic Leukemia and Ocular Lymphoma

A sixty-five-years-old male patient with history of non-Hodgkin’s lymphoma of the right orbit and a fifty-years-old female patient with chronic lymphocytic leukemia with peripheral lymphocytosis were treated on TomoTherapy. In the first case, a 1 cm beam was used for planning a delivery of 30.6 Gy in 17 fractions with a sequential boost of 9 Gy in 5 fractions. Tumor involvement of the ipsilateral lens and lacrimal gland in the planning target volume necessitated a compromise between preserving patient’s vision and delivery of tumoricidal dose to the target. In the second case, 30 Gy in 20 fractions delivery was planned with a 2.5 cm beam. The tumor extended anteriorly and this technique allowed sparing more orbital tissue and required a high modulation factor.

Fracture of the Hamate with Interposition of the Base of 5th Metacarpal, a Frequently Missed Injury

We report a case of hamate fracture in the coronal plane with interposition of the base of 5th metacarpal bone. This injury is frequently missed and may results in increasing rate of morbidity and mal-union. The Effects of the Film Thickness and Roughness in the Anodization Process of Very Thin Aluminum Films
