Effects of Gabapentin Enacarbil on Cortical Arousals, Heart Rate, Blood Pressure and Anterior Tibialis EMG Responses Associated with PLMs in Restless Legs Syndrome

Objective: This study was conducted to investigate the effects of gabapentin enacarbil (GEn) on sleep EEG, heart rate (HR), blood presssure, anterior tibialis EMG activity (PLMs power) and subjective complaints in subjects with moderate to severe RLS and disturbed sleep. Methods: This was a single site, single-blind, placebo run-in, fixed dose single group polysomnography (PSG) study. Eligible subjects (age 24-66 years) were treated with placebo for one week and GEn (600 mg/day) for 4 weeks. Two in-laboratory PSGs were collected for adaptation and baseline at the end of the placebo run-in period and for re-adaptation and efficacy assessment at the end of the 4-week treatment period. The primary endpoint was the difference in PSG derived cortical arousal intensity (arousal scale, 0-9) associated with PLMs between 4 weeks of treatment with GEn and placebo. Secondary endpoints included changes in HR responses (ΔHR), nocturnal systolic blood pressure (SBP) changes (>10 mmHg) secondary to PLMs and PLMs power. Other PSG and subjective measures were assessed. Results: Of 20 subjects enrolled, 18 completed the study. Subjects treated with GEn did not show significant improvements in cortical arousal intensity and ΔHR. However, subjects showed reduced PLMs power (p= 0.013) and associated reductions in nocturnal SBP per hour of sleep (p= 0.041) GEn showed significant improvement in other PSG parameters and subjective endpoints. Conclusion: The data suggests that GEn reduces the frequency and power of PLMs and the corresponding SBP changes in subjects with RLS. Despite reducing the total number of PLM associated arousals and nocturnal HR, the study did not demonstrate consistent effects of GEn on cortical arousal intensity and corresponding HR changes associated with PLMs. Clinical Trial Registration: ClinicalTrials.gov identifier: NCT02424695

No Evidence for Stress-Induced Sympathetic Activity in Young Women Classified as Restrained Eaters

Obesity has been linked to autonomic dysfunction, which is thought to be one of the main contributors for hypertension, cardiac remodelling and death. The aim of the present study was to investigate stress related changes in autonomous function in subjects at high risk for obesity (Restrained eaters). 33 healthy women aged 18-30 years were recruited through advertisement in local newspapers. Classification in restrained and unrestrained eaters was done according to norm tables from the German version of the Three Factor Eating Questionnaire. Participants were exposed to the Trier Social Stress Test, a standardized laboratory stressor. As an indicator of autonomous function the measurement of heart rate was performed. Restrained eaters did not respond with a significant increase in heart rate after stress. The results are interpreted with respect to consequences for nutritional intake and stress coping.

Bernoulli Equation in the Mitral Valve of Heart

In the past decade, finite element modeling of complete heart valves has greatly aided evaluation of heart valve surgery, design of bioprosthetic valve replacements, and general understanding of healthy and abnormal cardiac function. Such a model must be based on an accurate description of the mechanical behavior of the valve material. It is essential to calculate velocity/displacement, geometrical indices and strain rate/strain at a component level that is to work at the cellular level.

Assessment of Autonomic Function in Subjects Practicing Yoga Using Spectral Analysis and Approximate Entropy Method

Autonomic nervous system involvement in subjects practicing Yoga has rarely been studied and has shown conflicting results. Our main purpose was to assess the effect of Yoga on autonomic function of group of subjects practicing Yoga regularly in comparison with another group with normal control subjects by measuring the frequency gain response of the two groups. Other aim of this study was to determine wither the duration of Yoga practicing correlated with measures of heart rate variability signal (HRV) using approximate entropy index (ApEn).

The Use of Cardiac Resynchronization Therapy in Cancer Patients with Heart Failure

Investigate the use of cardiac resynchronization therapy (CRT) in cancer patients with heart failure (HF); assess factors associated with ischemic and non-ischemic HF. Many newer cancer therapies are cardiotoxic; thus, the incidence of HF has been increasing in this high-risk patient population. CRT has beneficial effects on morbidity, mortality, and left ventricular function in patients with non-ischemic cardiomyopathy, yet cancer patients and survivors who develop severe HF and are eligible for CRT often does not receive it.

Complex Chronic Patients and Atrial Fibrillation: Association with Cognitive Deterioration and Heart Failure

In developed countries, around 3-5% of the people could be identified as chronic complex patients, and they are increasingly at risk of atrial fibrillation (AF) and cognitive impairment. The main objective of this study was to present the current findings on the association between AF and cognitive impairment and mortality risk among chronic complex outpatients (CCP). A multicenter and prospective cohort study of mortality incidence was carried out from 1 January 2013 to 30 September 2016 in a sample of 932 adult patients registered as CCP. To predict hazard ratios, mean survival time, and survival probabilities, a multivariate Cox regression was used.
