Intravascular Large B-Cell Lymphoma, an Updated Review
Intravascular large B-cell lymphoma (IVBCL) is a rare but aggressive form of non-Hodgkin lymphoma. It is characterized by the malignant proliferation of lymphocytic tumor cells in the lumens of capillaries, small arterioles, and post-capillary venules. There exists two clinical variants: the Asian variant and the Western variant. The former is associated with neurologic and dermatologic signs and symptoms, whereas the latter often presents with splenomegaly, jaundice, and hemophagocytes. The Efficacy and Safety of Prophylactic Fluconazole Prevent Invasive Fungal Infection in Preterm Very Low Birth Weight Infants: An Update Meta-analysis
We identified 8 eligible trials that enrolled 1392 infants. There was significant effect of prophylactic fluconazole on fungal colonization (RR, 0.31, 95%C1:-0.24 to 0.41; P<0.00001). Effect of prophylactic fluconazole on IFI in VLBW infants: RR: 0.42(95%CI 0.23 to 0.76; P=0.004.). However, prophylactic fluconazole did not reduce mortality in VLBW infants (RR 0.76, 95% CI 0.58 to 1.01; P = 0.06). Studies did not report significant adverse reactions.

Chronic pain is a pandemic that has spawned the disproportionate opioid epidemic of overuse and overprescription in the United States. This challenge warrants review of physician education and curriculum to become more effective in pain management. Our current system for educating present and future physicians, as well as pain specialists, is flawed in a number of ways. The vast majority of physicians will encounter and manage chronic pain patients during their careers, but reviews have demonstrated dissatisfaction among medical students and practitioners with pain education and curriculum.
Liquid Biopsy in Advanced Gastric Malignancy and Molecular Targeted Therapy; a Case Report and Update Pertaining Anti HER2-Neu Therapy
Human epidermal growth factor receptor 2 (HER2) is responsible for the pathogenesis and poor outcomes of several types of cancers, including advanced gastric and gastroesophageal junction cancer. Molecular-targeted drugs on the other hand, such as trastuzumab, prolong overall survival and progression-free survival in HER2-positive gastric cancer. The purpose of the case report is to evaluate the impact of delivering trastuzumab in advanced gastric cancer with concomitant HER2 mutation and amplification.
