Detection of a Rheumatic Factor (RF,) Anti- Cyclic Citrullinated Peptides (Anti CCP) Antibodies and C- Reactive Protein (CRP) in Diagnosis and Prognosis of Rheumatoid Arthritis

Introduction: Rheumatoid arthritis ( RA)is an autoimmune and inflammatory disease. It is characterized by arthrosynovitis with resultant joint destruction . So, an early clinical diagnosis along with simple serological tests like anti CCP antibodies, C –reactive protein ( CRP) and Rheumatoid factor (RF) play an important role .An attempt was made to study and compare these parameters in OPD and IPD RA patients in our hospital setup, VMMC and Safdarjung Hospital , New Delhi. Aims and Objectives: To study the role of RF , Anti-CCP antibodies and CRP in early detection and prognosis of Rheu- matoid arthritis. Materials and Methods: A Total of 440 samples were studied from January 2018 to December 2020 (3years). Correlation between Rheumatoid factor(RF) ,anti CCP antibodies and CRP in suspected RA patients from various departments like PMR , Orthopaedics , Medicine, Obstetrics and Gynaecology and Paediatrics was attempted. RF and CRP was performed using latex agglutination test and anti CCP antibody by ELISA. Results and Conclusion: Of 440 samples tested, all the factors were positive in 87 patients . Anti CCP and RF alone was seen in 32 patients. RF and CRP positive in 49 patients, RF alone in 22 patients. In 250 patients all the tests were negative. So, Anti CCP along with RF and CRP showed a better positive predictive value in patients of Rheumatoid arthritis. But positivity of these parameters even in patients who lack sign and symptoms also aids in better diagnosis, treatment and prognosis.

Candida, a Rare Cause of Duodenal Perforation

Candida infections of the gastrointestinal tract are rare and perforation caused by Candida infections is extremely rare. Herein, we present a case of duodenal perforation where laboratory culture revealed the growth of a fungus, Candida and the absence of H. pylori from a 51-year-old female who presented with an upper abdominal pain while performing her religious obligation in the month of Ramadan. The results suggest emergency exploratory laparotomy and the perforation discovered was repaired through omental patching. An antifungal agent, Fluconazole, was administered.

Monoclonal Antibodies for the CIGB-552 Antitumor Synthetic Peptide Quantification

The CIGB-552 peptide is a synthetic peptide that exerts cytotoxic effect on tumor cells. Thus, CIGB-552 peptide quantification in patient samples is crucial for assessing the treatment efficacy. Therefore, this study describes the generation and characterization of monoclonal antibodies (mAb) directed against the CIGB-552 peptide to be used in a quantification assay in further pharmacokinetic studies in humans. In this sense, the CIGB.552-H294 mAb specificity was evidenced only against the CIGB-552 peptide and a metabolite of 17 amino acids resulting from the CIGB-552 peptide degradation detected in mouse sera.

Proteomic Analysis in Response to Germination in Ceiba pentandra (Kapok) Seeds

1-D, 2-D gel electrophoresis, mass spectrometry and bioinformatics analysis were established and utilized to examine the changes in protein expressions associated with germination of Ceiba pentandra seeds.The change in protein pattern was analysed by 1-D and 2-D gel electrophoresis for better separation and to ascertain the pattern of protein mobilization during germination.

Immunogenicity of Multiple Antigenic Peptides (MAP) Based on B and T cell Epitopes of E2 Glycoprotein of Chikungunya Virus in Murine System

Chikungunya is a viral disease caused by positive sense single stranded RNA virus. High fever, myalgia, arthralgia, body rashes are the characteristic features of Chikungunya transmitted by Aedes mosquitoes. We aim to demonstrate that dominant epitopes of envelope E2 protein can be used for diagnostic purposes as well as vaccine design. Multiple Antigenic Peptide (MAP) approach was used based on B and T cell epitopes of E2 protein. Humoral and cell mediated responses were studied in outbred and inbred H-2d mice. Different groups of mice were immunized intramuscularly with or without adjuvant (CpG ODN and murabutide) in PLGA microspheres. MAP entrapped in microspheres with CpG ODN showed highest IgG peak titer (2,98,000) with IgG subclass, mostly IgG2a/2b distribution. MAP’s antisera showed significant immunoreactivity with individual peptides. During thymidine incorporation assay for T cells, all the MAPs showed high lympho-proliferative response. Cytokine profile was dominated by Th1 and Th17 response. Thus, all the MAPs showed high humoral and cell mediated response with CpG/ murabutide adjuvant. MAP’s antisera showed significant in vitro neutralization of CHIKV strain in vero cells.

Gingival Stimulation: An Important Metabolic Regulator?

This study aimed to determine whether a relationship exists between gingival stimulation and the levels of leptin, ghrelin, insulin and glucose, which are important regulators of energy homeostasis. Blood samples for ghrelin, leptin, glucose and insulin were taken from 15 male volunteers (mean age 25.5±2.3 years; mean body mass index 24.4±2.79 kg/m2), who did not brush their teeth for one day, after a 12 h-long overnight fasting and before standard breakfast (0 min) and thereafter at 30, 60, 120 and 180 min after breakfast. After toothbrushing after dinner and after a 12 h-long overnight fasting, blood samples were taken again before standard breakfast (0 min) and then after at the same time points following tooth brushing.A significant reduction was found in the leptin levels measured at 0, 30, 60, 120 and 180 min after tooth brushing (p < 0.005). The ghrelin levels also declined at these time points but were significant at 0, 30 and 120 min (p < 0.05). Despite the reduced insulin levels at 120 and 180 min after tooth brushing (p < 0.05), no significant change was observed in the glucose levels.

Targeted Killing of Streptococcus mutans in Biofilms by a Pheromone Guided Antimicrobial Peptide HP30

Streptococcus mutans is a leading cariogenic pathogen of dental caries worldwide. Clinically, eliminating S. mutans from dental biofilms using antibiotics is not practical, because these agents indiscriminately kill other members of the resident flora, leading to ecological disruption and other negative clinical consequences. To develop target-specific antimicrobials, we evaluated several fusion peptides and identified a new peptide HP30 that showed a high selectivity for targeted killing of S. mutans.

Helicobacter pylori (HP) infection is present in more than the half of the world population. Most individuals are infected during early childhood; in developing countries, 50% of children are infected by the age of 5 years [1,2]. Studies suggest the main etiological role of HP infection in development of chronic gastroduodenal pathology and the formation of its most severe forms in patients of different age groups. However, not all infected people exhibit diseases associated with this bacterium. In addition, pathogenic mechanisms of duodenal ulcer disease (DUD) in childhood remain not fully understood.
