Successful use of Eltrombopag for Management of Temozolomide Related Severe Prolonged Thrombocytopenia and Aplastic Anaemia in The Treatment of Glioblastoma Multiforme (GBM) in a Regional Centre: A Case Report and Literature Review
Temozolomide is an orally administered chemotherapeutic drug used concurrently with radiation for the treatment of newly diagnosed glioblastoma. Although the addition of Temozolomide has shown to improve overall survival, it is commonly associated with haematological complications. Eltrombopag is an oral thrombopoietin (TPO) receptor agonist that is also used off-label for temozolomide-induced aplastic anaemia.
Anaemia Phenotypes in Highly Active Antiretroviral Treatment Defaulting Adults at the Comprehensive Care Clinic at the Siaya County Teaching and Referral Hospital
Anaemia is a major cause of morbidity and mortality among people living with human immunodeficiency virus (HIV) infection. While adherence to antiretroviral treatment (ART) reduces the incidence of anaemia, non-adherence (defaulting) to ART is associated with a number of haematologic derangements including anaemia. However, the mechanisms and predictors of anaemia have not been examined among ART defaulters in western Kenya. This study, therefore, aimed at evaluating the anaemia prevalence and classification of the anaemia based on haemoglobin (Hb) concentrations and erythrocyte morphology as well as suppression of erythropoiesis in ART-defaulting adults at a comprehensive care clinic in western Kenya.
Novel Alternative Treatment to Plasmapheresis in Evans Syndrome
Evans syndrome is a rare haematological disease characterised by concurrent occurrence of autoimmune haemolytic anaemia
(AIHA) and idiopathic thrombocytopenic purpura. The autoimmune haemolysis can lead to hyperbilirubinaemia, profound anaemia
and haemodynamic instability. It can be difficult to treat and when refractory to medical treatment it could necessitate emergency
splenectomy. Plasmapheresis is used in refractory haemolytic anaemia to aid removal of disease causing autoimmune complexes whilst
immunomodulators act to reduce autoantibody response and effectiveness. However, this is an expensive and resource intensive process
that is limited to tertiary referral centres. In this case study we present a successful novel alternative for life threatening AIHA in Evans
Syndrome using haemofiltration with on-going fresh frozen plasma transfusion when resources are limited or it is not an accessible
method.

We report a case of gross non-communicating hydrocephalus in a neonate of Congenital Rubella Syndrome which is a rare presentation.
