Late Benign Gastrobronchial Fistula after Esophagectomy for Esophageal Cancer: A Case Report
Benign gastrobronchial fistula (GBF) is extremely rare but highly fatal complication of esophagectomy for upper gastrointestinal system cancers. We present a benign GBF after three years esophagectomy for esophageal cancer with presenting recurrent pneumonia. We choose wedge bronchoplasty without lobectomy and primary repair of gastric conduit as an approach.

Ulcer with Fatal Perforation of A Gastric Blind Pouch: A Rare and Un-Expected Complication After A Salvage Reconstruction for Esophagus
We present a case of total esophageal reconstruction with a free Jejunal flap in a patient with a history of esophagectomy and two previous failed attempts at reconstruction. Despite the technique here described offers a reliable option when the stomach and colon are not available, the success of the reconstruction might be compromised by the onset of a perforated ulcer at the gastric blind pouch related to previous failed gastric pull-up (GPU) After a major reconstructive surgery of long hours, gastric pouch may develop ulcer with perforation and fatal empyema due to stress, although the severe complication is rare.
Timing is everything: Lipid and Energy Losses with Simulated Nasogastric Tube Feeds
To determine the proportion of human milk (pooled and homogenized) fat loss via feeding tube in a simulated 24-hour feeding cycle. Anonymous samples of frozen human milk were obtained, thawed and pooled (pHM) in a 1000 mL glass beaker with a magnetic stir bar. Eight syringes (30 mL each) were prepared and attached to 60” extension tubing and a 5 FR polyurethane or silicone nasogastric (NG) tube. Samples to test for pool uniformity were collected at the beginning, middle and end of syringe preparation. Milk was delivered sequentially via gravity, 30-minute or 60-minute infusion. Pooled human milk was analyzed for macronutrient content at the end of each feeding “cycle”. All scenarios were repeated using homogenized donor human milk (hDHM).
Gastric Partitioning Bypass Procedure: A Novel Technique in Bariatric Surgery
The current standard of care offers surgery as the most favored treatment modality for morbidly obese patients to achieve weight loss. The currently available surgical techniques have some limitations and introduction of novel techniques is inevitable. The aim of this study is to introduce the novel gastric partitioning bypass technique and assess its effectiveness and adverse effects.Thirty individuals were randomly recruited from a population who were eligible for bariatric surgery. The subjects had a baseline visit and were followed-up after surgery assessing weight loss and adverse effects related to the surgery.The study population included 30 patients (Mean age 41±5 and 93% female). Mean BMI and excess body weight at the baseline were 49±7 kg/m² and 79±15 Kg, respectively. Median follow-up time was 36 months and 27 (90%) patients completed the study. Excess weight loss was 54.76 ± 13.76 % and 60.43 ± 14.49% after 12 and 48 months, respectively. In post-operative period Two patients (6.6%) developed surgical site infection, other complications such as bleeding or anastomotic leakage, or any complication requiring emergent surgery were not detected.
Methylmalonic Acidemia and Megaloblastic Anemia due to Congenital Intrinsic Factor Deficiency
Causes of vitamin B12 deficiency in children include decreased intake, abnormal absorption, and inborn errors of B12 transport and metabolism. Rare causes of abnormal cobalamin absorption include Imerslund-Grasbeck syndrome (IGS) and intrinsic factor deficiency (IFD). IGS and IFD are caused by defects in the genes CUBN, AMN, and GIF. We describe a 2 year old male who presented with severe megaloblastic anemia and methylmalonic acidemia. He was found to have a GIF heterozygous mutation c.79+1G>A associated with congenital gastric intrinsic factor deficiency and a novel variant c.960C>A in trans position.
Liquid Biopsy in Advanced Gastric Malignancy and Molecular Targeted Therapy; a Case Report and Update Pertaining Anti HER2-Neu Therapy
Human epidermal growth factor receptor 2 (HER2) is responsible for the pathogenesis and poor outcomes of several types of cancers, including advanced gastric and gastroesophageal junction cancer. Molecular-targeted drugs on the other hand, such as trastuzumab, prolong overall survival and progression-free survival in HER2-positive gastric cancer. The purpose of the case report is to evaluate the impact of delivering trastuzumab in advanced gastric cancer with concomitant HER2 mutation and amplification.

Preeclampsia is clinically defined by hypertension and proteinuria, with or without pathologic edema that occurs after 20 weeks’ gestation, but can also present up to 4-6 weeks post-partum.
