Simultaneous Tuberculous Infection of Lung and Allograft in Renal Transplant

Tuberculosis is common infectious complication in kidney transplant recipients. In immunosuppressed patients, clinical manifestations of tuberculosis are varied and delayed diagnosis and poor clinical outcomes. Especially allograft involvement of tuberculosis can cause allograft loss. In this report, we present the case of 46-year-old man diagnosed disseminated tuberculosis involving allograft kidney successfully treated with maintenance of allograft function.

Comparative Simulation Study of Direct and Indirect Target Production of 99Mo with Linear Accelerator at SAMEER

When high energy electron beam falls on a high Z target it generates bremsstrahlung photons which can be used to produce 99Mo from 100Mo using photo-neutron (γ, n) reaction. Two different approaches are studied to carry out photo-neutron reaction. First, a converter target approach in which photons are generated in a high Z target of tungsten using (e,γ) reaction and then (γ, n) reaction in 100Mo for 99Mo production. Second, a direct target approach wherein 30 MeV electrons hit directly onto 100Mo target to carry out both (e, γ) and (γ, n) reactions in the 100Mo target itself. GEANT4 simulation gives photons and neutrons fluxes and their angular distributions from both the approaches, which are then used to calculate activity of 99Mo. The study shows that for very thin 100Mo target of about 0.1 radiation length converter target shows better activity whereas, for any thickness beyond it the direct target approach is found to be more efficient. For 30 MeV and 10 kW beam falling on 100Mo target with 1.4 radiation length, direct target approach is found to be more efficient. Therefore, after improvising the geometry of 100Mo target in the case of direct target method an increased specific activity is obtained through GEANT4 simulation.

Predictive Value of ERCC1 Expression on Treatment Response to Cisplatin-Based Regimens in Small Cell Lung Cancer: A Retrospective Analysis

We aimed to determine whether the excision repair cross-complementation group 1 (ERCC1) expression predicts response to cisplatin-based chemotherapy in patients with small cell lung cancer (SCLC). This was a retrospective study, in which hospital files of 31 patients (29 males, 2 females; mean age, 62.26±7.71 years) were reviewed. All patients were treated with etoposide+cisplatin. The lung biopsy and mediastinal lymph node samples were applied anti-human monoclonal antibodies against ERCC1: immunohistochemical staining was considered positive if 25-100% of the cells showed nuclear staining, and negative if less than 25% of the cells showed negative or slight staining. Response to treatment was evaluated as regression (complete or partial response), progressive disease, or stable disease. Of the patients, 18 (58.1%) had limited-stage, and 13 (41.9%) had extensive-stage SCLC. Median follow-up duration was 15 months (range: 1-60 months). ERCC1 staining was positive in 9 of 31 patients (29.0%). In 12 patients (38.7%), tumor regression (complete or partial response) was obtained after chemotherapy. Tumor regression rate was higher in ERCC1 positive patients than those with negative ERCC1 expression (66.7% vs. 27.3%, p=0.036). In conclusion, high expression of ERCC1 was associated with higher response rate to cisplatin-based regimens in patients with SCLC.

microRNA 181a-5p Reprogramed Glucose and Lipid Metabolism in Non- Small Cell Lung Cancer

Cisplatin-Gemcitabine Related Cardiomyopathy in Non-Small Cell Lung Cancer NSCLC Patient: A Case Study

Gemcitabine is a pyrimidine analog and cisplatin is a platinum agent, they are usually combined to form a chemotherapeutic doublet used to treat different types of oncological cancers, including non-small cell lung cancer (NSCLC). Although these agents are not known to cause such events, few cases related to possible cardiotoxicity side effects have been reported ; including angina, chronic heart failure, arrhythmias, and cardiac ischemia. A 52-year-old African male presented with Stage IV lung adenocarcinoma metastatic to the bone, pleura and lymph node, EGFR wild type. Patient has a history of hypertension, diabetes, and atrial fibrillation and on rate control medications. He was treated initially with cisplatin-gemcitabine, after which he developed cardiomyopathy.

Benign Pulmonary Metastasizing Leiomyoma

Benign metastasizing leiomyoma (BML) is a rare event that occurs in middle aged women with a history of uterine myomas. It is characterized by multiple leiomyomatous lesions in many tissues and organs, especially in the lungs. We report the case of 56 years old woman presenting chronic cough, hemoptysis and having a history of myomectomy for a uterine leiomyoma 6 years ago. Chest CT showed a right pulmonary process. The histological diagnosis was defined by bronchoscopy and biopsies. The surgery was not an option because of the bronchovascular extension of the tumor, so hormonal therapy was administrated to the patient. One year later, Chest CT showed that the tumor doesn’t increase.

Scalp, Skull, Lung and Bone Metastasis from Follicular Thyroid Carcinoma - A Rare Case Report

Concurrent scalp, skull, lung and bone metastasis from follicular thyroid carcinoma is a very rare event. We herein present the case of a 37- year’s old lady, who presented to us with huge scalp swelling that proved to be metastasis from a follicular thyroid carcinoma. The metastatic workup disclosed bilateral lung metastasis and additional metastasis in right iliac crest and 6th rib. The course of her disease was relentless. Although follicular thyroid carcinoma has an excellent prognosis, the presence of metastatic disease leads to a very dismal prognosis.

Macrocystis of the Lung in a Preterm Neonate: Case Report

Thoracic ultrasonography has been used to evaluate pulmonary parenchyma and the macrocystis of the lung in a preterm with congenital cystic adenomatoid malformation type I. The images gained have been important for diagnosis and therapeutic strategies in our case. An infant was prematurely born with congenital cystic adenomatoid malformation type I. The chest X-ray and computerized tomography showed a great opaque area in the entire right lung, we considered performing a pneumonectomy. This opaque area hampered the expansion of left lung. After draining the macrocystis by ultrasonography-guided puncture, it was possible considering and performing the lobectomy due to the reduction of the volume of macrocystis. During follow up at 4 months, the infant did not show complications and the chest X-ray revealed the reduction of volume of the dense area in parenchyma of right lung.

Double Stroke and Double Malignancy: A Case Report

Stroke associated with malignancy is rapidly increasing. Lesion types mainly resemble cardio embolic multiple ischemic lesions. We present a rare stroke patient with both ischemic and hemorrhagic lesions simultaneously. The etiology of stroke was found to be due to lung cancer and essential thrombocytosis. Differential diagnosis in this case can help for further investigation of strokes with malignancy.

Lung and Parietal Hydatid Disease

Hydatid disease, which is also known as cystic echinococcosis, is a zoonotic infection caused by the cestode tapeworm Echinococcus granulosus and rarely by Echinococcus multilocularis. In this report we describe an unusual case of a 75-year-old man who was admitted to our hospital for a mass extending from the lumbar region to the right cervical region. Computed tomography and MRI revealed a huge cervical and thoracic mass of the right chest made of multiple cystic lesions.

Pulmonary Adenocarcinoma Transforming into Small Cell Carcinoma: An Extreme Rarity
