An Accidental Intrathecal Injection of Tranexamic Acid: A Never Miss Event
Several factors have been identified as contributing to medical errors such as labels, appearance, and location of ampules. In this paper, inadvertent intrathecal Injection of tranexamic acid has resulted from similarities in appearance between tranexamic acid and heavy bupivacaine 0.5% ampoules. The patient experienced severe itching in the gluteal region followed by generalized myoclonic seizures after accidental intrathecal administration of tranexamic acid.

Systematic literature review was conducted. Criteria included seizures, status epilepticus, and/or CMA developing prior to or during hospitalization, with concomitant COVID-19. COVID-19 severity was dichotomized into mild and severe cases, based on severity of respiratory symptoms. Good outcome was defined as discharge without severe deficits
and/or return to near baseline.

A rare case of idiopathic temporomesial encephalocele in a 64 years old female is presented. The clinical symptoms of this adult patient were spontaneous rhinorrhea and headaches. A temporomesial encephalocele could be diagnosed and the cele was removed surgically in two operative procedures.
