Background and Objective: Septic shock remains an impatient cause of morbidity and mortality. Early restitution of the circulation improve tissue oxygen delivery and increase survival. This study is a prospective randomized single center study its main objective is to investigate the therapeutic value of hypertonic saline in patients with septic shock. Methods: Fifty-two critically ill patients admitted with septic shock divided into two groups, the first group received isotonic saline (control group) starch, while the second received hypertonic saline 5%. Results: The mean central venous pressure in the first and third hours of the intervention in the isotonic saline group was significantly higher than the hypertonic saline group. (Respectively, P=0.024 and P=0.014). The average pH and BE in the second and third hours of the intervention in the isotonic saline group were significantly higher than the hypertonic saline group. (Respectively, P=0.032 and P=0.040). The average bicarbonate in the third hour of the intervention in the isotonic saline group was significantly higher than the saline group. (P=0.008). Also, the average optic nerve diameter in the third hour of the intervention was 4.4±0.1 in the isotonic saline group and 4.2±0.2 in the hypertonic saline group, and there was a significant difference between the two groups in this hour. Conclusion: Our study results showed that hypertonic saline administration in septic shock patients is safe and can be used as an adjuvant resuscitation fluid therapy as i it has a preventive ability in causing tissue edema compared to isotonic crystalloid solutions.

Introduction: Hemorrhagic cerebrovascular accident (HCVA) is a medical or medico- surgical emergency requiring treatment in neurovascular or intensive care units. The aim of this work was to determine the prognostic factors of hemorrhagic stroke in young subjects in intensive care in the 3 university hospitals of Abidjan. Methods: This is a descriptive and analytical retrospective study covering, which extends from January 2018 to December 2022, taking into account. All patients aged 18 to 50 admitted to intensive care for a hemorrhagic stroke during the study period. Results: 146 patients including 100 men were included. The average age was 43 years old. High blood pressure was the main modifiable risk factor. The average Glasgow score was 8.2 and 98% of cases presented with impaired consciousness. Grade 3 hypertension was found in 73 patients on admission. The main location of the hemorrhage on imaging was parenchymal and 45% of subjects had an ICH score greater than or equal to 3 in almost 50% of cases. Nicardipine was the antihypertensive of choice. 103 patients were intubated and ventilated. 52% underwent neurosedation 13% underwent neurosurgery. The average length of hospitalization was 8.3 days during which 72.6% of patients presented complications, mostly infectious. Mortality remains high at 66.44% of cases. We listed as prognostic factors, inaugural deep coma, inaugural grade 3 arterial hypertension, ICH severity score greater than or equal to 3, renal failure and length of stay less than 7 days. Conclusion: Hemorrhagic strokes in young subjects remain a common condition in our country. Mortality in intensive care is high and is linked to clinical and neurological severity, biological disorders, the occurrence of complications and the length of stay in intensive care

Only 10% of people diagnosed with multiple myeloma are under 50, and 2% of people are under the age of 40. Here we pre sent the case of a 24-year-old female admitted to the hospital for recurrent genital infections, severe anemia and a normal phosphocalcique balance. The myelogram studies revealed normochromic normocytic non-regenerative anemia, as well as a 12% marrow infiltration by abnormal plasma cells. Immunological analysis by immunofixation revealed a homogeneous band in the gamma-globulin zone. After the 5th chemotherapy cycle, the 6-month follow-up assessment showed complete remission.

There is growing evidence suggesting a correlation between multiple copies of transcription factor-3 (TCF3) and various malignancies, yet its impact on patients with breast cancer remains uncertain. The present study utilized the Cancer Genome Atlas (TCGA) database, Gene Expression Omnibus (GEO) database, and the UALCAN database to obtain expression profiles and clinical data from patients with breast cancer. A comparative analysis was performed to assess the differential expression levels of TCF3 in breast cancer tissues as compared to normal breast tissues. TCF3 was found to be overexpressed in breast cancer and exhibited significant associations with T stage, age, race, PAM50 subtype, histological type, ER, PR and menopause status. Subsequently, analyses were performed to determine functional enrichment. The enriched pathways encompassed xenobiotic metabolism by cytochrome P450, chemical carcinogenesis-DNA adducts, extracellular matrix organization, cornified envelope formation, collagen biosynthesis and modifying enzymes, as well as immunoregulatory interactions between lymphoid and non-lymphoid cells. Single-sample gene set enrichment analysis (GSEA) was utilized to evaluate the infiltration of immune cells. The methylation status of TCF3 was analyzed using the UALCAN and MethSurv databases. The overexpression of TCF3 is a significant contributor to decreased disease-specific survival (DSS) and progression-free interval (PFI). Likewise, the low methylation status of TCF3 is also associated with an adverse prognosis. Finally, the differential expression of TCF3 between tumor and normal tissues was validated using data from multiple sources, including GSE22820 dataset, 49 pairs of tumor and paracancerous samples sequencing data, and CPTAC dataset. Moreover, proteomic analysis of the CPTAC dataset showed that TCF3 expression was associated with alterations of multiple tumor-related pathways. Therefore, TCF3 may serve as a new tumor-related biomarker.
